The basic format includes the content provided in the Blue Button electronic health record, as well as additional helpful features.  In keeping with the patient centric focus, content on the About Me page will be able to be changed by the patient, in case name, marital status, religious affiliation, address, phone, and spoken languages change.

A scroll bar on the left reveals the chart page choices for Guardian, Provider, Allergies, Immunizations, My Meds, Plan of Care, Encounters, Problems, Procedures, and Lab Results. More pages can be added as needed, including a Nutrition page, and a Health Ed page that could host links to health education reading on specified topics and links to health game apps prescribed, Game Rx, such as Max’s Plate, Food Focus: Fruits, or Breast Cancer Care. 

The My Progress page may include a series of slide bars that a person can set to how they are doing on a number of health parameters to show their doctor or caregivers, including sleep, diet, sun, meds, exercise, BMI, etc.

The bottom tool bar hosts a place to log questions to pose to the health team, possible translations, information about the app use and support contact, as well as email form to send to persons of choice.
